PLA has a wonderful site with a nice Speaker Confirmation Letter.

Here is a simple one that I sent out to speakers:

Dear Speaker;

Thank you for agreeing to speak at the Annual Conference of the Utah Library Association. The program is scheduled for________ <day of week>,________ <Month, date> from ______<time> in Room ___<?> at the Eccles Conference Center in Ogden. Your co-panelists are __________<fill in>. Your remarks should cover the following topic: ____________<title of program>. Our panel will be most interesting if we each cover areas as different as possible. Please send me a rough outline on what you plan to cover by February 1, 2000. I will distribute this to all the panelist and then we can see what areas are missing or over covered. The focus audience is paraprofessionals. I will also need an introduction for the moderator. I am not sure who will introduce and will let you know as the time comes closer. No equipment has been reserved so if you wish something, please let me know soon. The order I would like to follow (after the moderator's introduction) is: _______________<list order of speakers>. Please let me know if you have any other preferences. As we have approximately 45 minutes (30 minutes for questions afterwards, introductions, etc.), each speaker will be limited to 15 minutes.
